There are still appointments available for Mendham Township's COVID-19 Vaccination Clinic today at the Mendham Township Middle School (6 Washington Valley Rd, Brookside).

Pfizer's COVID-19 Vaccine will be administered at this clinic for anyone 12 or older. Appointments

are still available from 3:30pm - 7:30pm.

There are walk-in appointments available as well.

Attached to this email are the registration forms. Please bring a valid photo ID and copy of your health insurance card to the appointment. If you do not have health insurance, you can still receive the vaccine with no charge. Masks and social distancing are required at the vaccination site.
